• Applicant must complete the LRFC Application  and Pilot History form and return with a check made payable to LRFC in the amount of $3,000.
• If there is a waiting list, return with a non-refundable check of $100 to reserve spot on waiting list.
• Along with application, a photocopy of the applicant's certificate, medical, and last two (2) logbook pages.
• Return all to Operations Manager for review, reference check, and presentation to the Board of Directors:
